EMPOWR

Embracing Progress and Opportunities, We Rise. Using a team-based approach, EMPOWR supports youth ages 17-25 in navigating the transition to adulthood. 

TEAM-BASED CARE FOR EMERGING ADULTS

Preparing to live independently is a challenge for all teens. Youth leaving foster care and/or experiencing social and emotional challenges often do not receive the support they need to succeed. Young people that choose to participate in the EMPOWR program have a core team of providers that work together to help the participant reach the goals they identify as most important. Our coordinated approach helps to minimize youth stress and burnout that can result from multiple competing demands, navigating fragmented systems, and other barriers to success.

SUPPORTING YOUTH

When a young person becomes an EMPOWR participant, they are introduced to our team, which includes a care coordinator, case manager, peer partner, therapist, and family partner.  The participant then works with team members to identify their goals, strengths, and what is going well. The youth can meet with any member of their team, or the whole team, as often as they want. We work together to coordinate services and help the participant access community resources, so that the youth can focus on meeting their goals rather than managing multiple providers and systems. If the participant is interested, EMPOWR also encourages them to include supportive people in their lives– such as family members, teachers, or coaches – to be part of their support team. In addition to our youth-driven individual services, we also hold monthly workshops for participants to connect with each other and get help with a variety of topics, such as managing finances and improving self-care.

  • Addressing housing concerns

  • Connect to resources

  • Build and maintain healthy relationships

  • Improve emotional wellbeing

  • Navigate the healthcare system

  • Increase financial literacy

  • Get parenting and family support

  • Access holistic services, such as acupuncture or massage through our partnership with Freedom Community Clinic

  • Connect with other young people​
