CARE COORDINATION
Our ICC team works with youth who have multiple providers or change placements often to build community, shine light on dim pathways, and turn possibility into collective vision. We collaborate with youth so they don't have to navigate challenges alone.
OUR APPROACH
Team-based care is at the heart of all of our services, reflecting our value of community and connection. The children we see are often involved in multiple public systems that impact their lives, such as child welfare, education, or probation. Our teams partner with youth and families to advocate for what they need—ranging from increased educational support to stable housing. Through collaboration with important people in the youth’s life, such as relatives, teachers, and other professionals, we build a team that has a common understanding of a child’s strengths, hopes and goals.

INTENSIVE CARE COORDINATION
Youth in the child welfare system that have lived in three or more foster homes within two years, and who have multiple services providers are eligible for Intensive Care Coordination (ICC) services. Our care coordinators support young people in navigating complex and disjointed service systems to ensure that the youth’s goals and choices are the top priority. Through Child and Family Team (CFT) meetings, youth work with their team to build consensus on goals, navigate barriers, and track progress along the way.
When new challenges arise, such as changes in school or figuring out where a child is going to live, we make sure the youth’s hopes are front and center. Our care coordinators ensure that adults in the child’s life are aligned on the next steps towards helping the child achieve their goals. This includes helping families develop skills to work through disagreement and relationship challenges that are getting in the way. Through every decision and conversation, we elevate the young person’s strengths and resilience, which are often discounted when the immediate focus is on resolving a crisis situation. The overarching goal of ICC services is for youth to have a stable living situation, connections to natural supports, and teams and systems that collaborate effectively on their behalf.

All youth and young adults who currently receive mental health services from WestCoast Children's Clinic are screened to identify eligibility for this service. Criteria includes: foster placement or housing instability (3 or more changes within 2 years), have full scope Medi-Cal, have multiple systems of care or service providers, have complex mental health needs, are under age 21.
